Health & Social Services Coordinator
About The Role
The Health & Social Services Coordinator supports whānau with complex needs to access, navigate and stay connected with health, social care and support services.
Working with clinical teams, Mission kaimahi and external agencies, the role bridges health and social services through engagement, advocacy and coordination.
This front-facing role requires professionalism, sensitivity, respect and confidentiality when working with patients, Mission kaimahi and external providers.

What You Will Be Doing
- Coordinate health and social care for whānau with complex needs, especially those at risk of disengaging or being lost to follow-up.
- Provide time-limited key-working and social support where no other suitable key worker or service is in place.
- Support whānau to navigate primary, secondary and community health services, including priority referrals, hospital discharge and outpatient care.
- Act as a coordination bridge between Calder Health, Street to Home, Supportive Housing and other Mission services.
- Participate in multidisciplinary hui, case reviews and health reviews to support improved health and social outcomes.
What We Are Looking For
- A relevant tertiary qualification in health, hauora, social services, community services or a related field, plus at least three years’ experience in health navigation, social services or community-based support.
- Capacity to demonstrate through actions commitment to Te Tiriti o Waitangi and the Mission’s values of Manaakitanga, Atawhai, Rangapū and Mana Tika, Mana Ōrite.
- Experience supporting people with complex health and social needs, including homelessness or housing instability.
- Strong skills in advocacy, communication and building trusted relationships.
- Ability to remain calm and professional in distressing, emotional or complex situations, including occasional aggression.
